1. One cover for each title you are submitting. (please note, covers are not returned)

2. Place a small white sticker in the upper right hand corner of the cover with the wholesale price.

3. Print your name and a telephone number where you can be reached during the day on the back of

    each cover.

4. Please do not submit complete pattern packets. We will request these at a later date if needed.
